• Statement letter of school transfer from previous school and legalized by Dinas Kota/ Kabupaten in previous school area
• Copy of previous school’s “Accreditation Certificate” or can be replaced by permission letter of school operation that are legalized (only for private school)
• Copy of legalized Format 8355
• Letter of “Good Conduct” from previous school
• Original copy of school report card from previous school (will be returned later on after change of school process by Dinas Kota)
• Copy of school report card* (at least the last 3 years) and legalized
• 4 latest color photos (3×4 cm)
• Copy of Birth Certificate
• Copy of Family Card
• Copy of parents’ ID
• NISN (Nomer Induk Siswa Nasional)
• Medical Records

International Students

• Letter of validation from Indonesian Ministry of Education
• Statement letter of school transfer from previous school and legalized
• 4 recent color passport photos (3×4 cm)
• Copy letter of “Request of School Transfer” from parents
• Original copy of school report card from previous school (will be returned later on after change of school process bu Dinas Kota)
• Copy of school report card* (at least the last 3 years) legalized
